We noticed a fall in numbers of completed trial forms. Based on research felt that the main issue was the form appeared too long — and because of that people were discouraged to complete it. With the goal to increase completed forms in mind, we divided the questionaire into 3 categories: user contact information, user job function and what the user was looking for/their goals.

We then divided the page into 3 separate “tabs”—far more visually digestible than the original layout (as seen below.) After implementing this new segmented style, we had an increase of 2x the amount of forms completed.
